I acknowledge that a tattoo is a permanent change to my appearance and that Dark Omen Tattoo/ artists involved are not responsible for later changes/removal of said tattoo. I am not pregnant or nursing. Nor am I under the influence of drugs, alcohol, or any other substance. I acknowledge I am over the age of 18 at the time of my tattoo and have truthfully represented to my artist proper identification. I agree that I have truthfully represented that obtaining a tattoo is my own choice and my choice alone. I agree to the application of the tattoo and to any actions or conduct of, Dark Omen Tattoo/ artists associated reasonably necessary to perform the tattoo procedure. I agree to all photography/ videography reasonably associated with the tattoo session. Granting complete ownership and control of these photo materials to Dark Omen Tattoo/ artists associated. To my knowledge, I do not have any mental, medical, or physical impairments or disabilities that may affect my well-being as a direct result of my decision to obtain a tattoo. I do not have any skin conditions such as acne, scarring, eczema, psoriasis, sunburn, etc in the area being tattooed. If I have any infection or rash anywhere on my body I will inform my tattoo artist. I understand that if I receive any skin treatments, laser hair removal, or other skin alerting procedures; or experience significant weight gain/loss, it may result in changes to my tattoo. I agree with the tattoo's design placement, color, and size. I understand the original design may not always be the finished product. I acknowledge that variations in color and design may exist between the tattoo design selected and the tattoo applied to my body. I understand that pigment/ ink vibrancy may vary on different skin tones. I acknowledge it is not reasonably possible for Dark Omen Tattoo/ artists associated to determine if I might have an allergic reaction to the pigments/inks or process used in/ on my tattoo. I accept the risk of any possible reaction. I acknowledge the risk of infection is always possible as a result of obtaining a tattoo, particularly if I do not take proper care of my tattoo. I acknowledge that I have received tattoo aftercare instructions, and agree to follow the instructions precisely. I accept the risk of possible infection. I agree that any touch-ups needed due to my own negligence will be at my own expense. I acknowledge and agree that I have been presented with any single-use needles in a sterile and sealed package that will be used at the time of my tattoo appointment, I agree that I witnessed the opening of said needles in my presence. I acknowledge and agree that I have been informed of proper ink-handling procedures and that only these procedures will be used at the time of my tattoo. I have been given the full opportunity to read this form. I acknowledge I have been given the full opportunity to ask any questions about this form.
